FAILURE ANALYSIS OF ROD VALVE ACTUATOR 15-UV-023 (CATALYST VALVE) IN UNIT RCC

Base on the laboratory examination/assessment results together with document file information, the material of Rod Valve Actuator 15-UV-023 (catalyst Valve) was not due to material inadequacy. The main causing of Rod Valve Actuator's failure was cyclical abnormal overloads. The hard friction of disc and set scratch in the system of catalyst valve system was caused the overload factor. The second causing of Rod Valve Actuator's failure was erosion by intrusion of catalyst material of catalyst between the space of disc and set scratch.
